Veterans and service members are required to complete one or more of the following forms in order to receive VA education benefits.

We highly encourage the online submission of the initial application for VA education benefits using the Vets.gov or VONAPP system.

General Information Sheet
This IUP-specific form must be completed by every applicant. This form contains general information that we are required to have on file.

VA 22-1990 — Application for VA Education Benefits
This form must be completed by any veteran or reservist seeking benefits for the first time. The form must be completed online using the Vets.gov website.

 

The following forms may be necessary depending upon an individual student's unique circumstances.

VA 22-1995 — Request for Change of Program or Place of Training Form
If you, the veteran or reservist, are planning on transferring to or from another institution of higher learning, or if you are planning to change your major at IUP, you will need to complete this form. The form must be completed online using the VONAPP system.

VA 22-5495 — Request for Change of Program or Place of Training - Dependent Student
If you are a dependent or the spouse of a deceased or disabled veteran and are planning on transferring to or from another institution of higher learning, or if you are planning to change your major, you will need to complete this form. The form must be completed online using the VONAPP system.

VA 22-5490 — Application for Survivors' and Dependents' Educational Assistance
If you are the son, daughter, or spouse of a deceased or disabled veteran and you wish to apply for Chapter 35 (Dependents' Education Assistance Program) benefits, you will need to complete this form. The form must be completed online using the VONAPP system.
