It is the policy of the Health Service not to provide medical excuses. In certain circumstances a Verification of Illness form may be provided; however, the Health Service does not have the authority to excuse students from attendance policies determined by individual professors.
Students are encouraged to correspond directly with the faculty as soon as possible regarding their absence.
Information about a student's visit is confidential and will not be provided to anyone without the permission of the student.
Under no circumstances will the Health Service provide a Verification of Illness form for an illness that we did not provide care for.
