Student Billing Office Forms

A 1098-T form is used as a guide in the preparation of your taxes. Its purpose is to assist you in determining your eligibility for an education tax credit.

Complete the form and email it to the Office of Student Billing at student-billing@iup.edu. 

A $40 employer reimbursement plan fee will be added to your account. Please log in to your EasyPay account and submit payment for this fee along with any remaining balance due. Any portion not paid, less the amount the employer is paying, is subject to late fees.

Please submit a new form each semester.

What are federal Title IV funds?

Title IV funds are federal student aid funds, which are from federal student aid programs administered by the US Department of Education. The US Department of Education regulations only allow schools to use your Federal Student Aid to pay for current academic year institutional charges. However, you can allow the use of surplus financial aid to pay for non-allowable charges that may be on your account.

Title IV funds include:

• Direct Subsidized/Unsubsidized Loans
• Direct Graduate PLUS Loans
• Direct PLUS Loans
• Federal Pell Grants
• Federal Supplemental Educational Opportunity Grants (SEOG)

Federal Title IV funds do not include scholarships from the university, the state, or other private organizations.

Allowable charges:

• Tuition
• Mandatory fees
• Housing and food, if contracted with the University

Non-Allowable charges:

• Health Center charges
• Library fines
• Other charges

How are Title IV funds applied to my account?

The U.S. Department of Education requires that Title IV funds be applied to specific allowable charges. Allowable charges include: tuition, mandatory fees, and housing and food billed by the University. If your total of Title IV funds exceeds the total of these qualifying charges, the University must refund that excess to you unless you give permission to do otherwise.

Does the authorization need to be completed each year?

No. The authorization remains in effect while you are a student at IUP. It may be rescinded or changed by sending a written request to the Student Billing Office.

What does the authorization say?

“I authorize Indiana University of Pennsylvania (IUP) to apply my Federal Title IV Financial Aid to any non-allowable charges on my student account.”

• "Authorize” indicates federal funds can be used to pay for other charges, such as student health center visits, library fines, etc.
• "Decline” indicates federal funds will not be used to pay, and the student will be liable for all charges not covered by financial aid.

What if I don’t grant the authorizations?

If you do not grant authorization, your federal financial aid and loans cannot be used to pay for charges other than tuition, fees, university housing, and meal plans, and any excess aid will be refunded to you. Therefore, you may receive a refund and may simultaneously be sent a bill for any charges that could not be paid with your Title IV aid. Receiving a refund does not indicate that there are no pending charges on your account.

Direct Deposit Cancellation Form

• If you wish to discontinue the direct deposit of financial aid refunds, please log in to MyIUP and inactivate your current banking information.
• You can update your banking information by logging in to your MyIUP account.

Request Stop Payment of Refund Check

• To request a stop payment, please stop by our office or email us from your IUP email address. Your request should indicate that you are requesting a stop payment of a refund check. Please include the amount and date the refund was processed, as well as your banner ID.
  
  Note: By doing this, you understand that after you initiate this request, a stop payment will be placed, and if you find the refund check, you will not cash it. IUP will reissue the refund check after the original has been stopped and voided.
