      Use this form to request an exemption for the Wellness Fee.

  • 1098T Information
    Form 1098T is used as a guide in the preparation of your taxes. Its purpose is to assist you in determining your eligibility for an education tax credit.
    Direct Deposit Cancellation Form
    Direct deposit cancellation forms are no longer necessary. If you wish to discontinue the direct deposit of financial aid refunds, please utilize URSA to do so.
    Employer Tuition Reimbursement Form
    Complete this form and submit it along with any remaining balance due to the Office of Student Billing. Please download and submit a new form each semester.
    Tuition Waiver
    Information on tuition waivers and tuition savings account program.
    Title IV Authorization
    By completing this form, you are giving permission to the university to pay any outstanding balance on your student account (including non institutional charges) for the same aid year.  This will not cover charges from prior aid years.
    Stop Payment Request Form
    This form needs to be completed any time a student wants a stop placed on an outstanding check.
