PEBTF’s Get Healthy Know Your Numbers wellness screenings take place July 1–December 31.

- Spouses are not required to complete a wellness screening
Registration for your Get Healthy Know Your Numbers wellness screening is open for employees enrolled in PEBTF medical and/or prescription drug benefits. Complete your wellness screening by December 31, 2025, to qualify for the savings for July 1, 2026, through June 30, 2027.
Register early to make sure you get an appointment at our wellness screening event. The appointments can fill up fast!
To Make an Appointment:
Register Online
- Go to PEBTF.
- Click on the Get Healthy logo on the left side of the home page and follow the instructions to go to the Quest website. You need to include an email address.
- You can sign up to receive text message appointment reminders, in addition to email reminders. Opt in during the online registration process or update your preferences anytime in the My Account section of the Quest website.
Register by Phone
If you do not have email or internet access, call Quest Diagnostics to register for a wellness screening or to request a Physician Results Form at 1-844-598-7966, Monday–Friday, 8:00 a.m.–8:00 p.m. EST, and Saturday, 8:30 a.m.–3:00 p.m. EST.
Three Ways to Complete a Wellness Screening

Onsite events (fingerstick blood draw):
Please schedule an appointment and arrive at your scheduled time. Walk-ins will not be accepted this year, nor will same-day requests to change your appointment time. If you are coughing, sneezing, or have a sore throat, protect yourself and others by wearing a mask. If you are experiencing COVID-19 symptoms, do not attend an on-site event. -
Quest Diagnostics® Patient Service Center (venipuncture blood draw):
Make an appointment and don’t schedule as a walk-in to cut down on wait time. Do not wait until the deadline, because Patient Service Centers may be closed on December 31 for the holiday. -
Your doctor’s office (venipuncture blood draw):
Download the 2025 Physician Results Form from the Quest Diagnostics website. It will populate with your demographic information. Do not use an old form, because your results will not be recorded! A Physician Results Form must then be completed by your doctor in its entirety. You must upload or fax the completed form to Quest Diagnostics by December 31, 2025.
