To sign up for a summer meal plan, please fill in the following form.

An e-mail with the following information will be sent for processing. You may also print this form, sign it, and return it to the Office of Housing, Residential Living, and Dining (where you can also pick up a paper copy if needed).

Note that only off-campus students should use this form. On-campus students will specify their meal plans on their summer housing applications.
