Each candidate must submit a Personal Statement and Vita focusing on the last five years. All application packages should also include a Table of Contents for the supporting materials addressed below:

While the strongest applications are likely to address more than one category, please note that it is not necessary to address every category. For teaching award applications, five (5) years of student evaluation summaries and peer observations are required.
