- Supervisors of employees in equalization units must complete the workbook and submit it to the Office of Human Resources at the completion of the six-month period.
Statement of willingness to work overtime form
- This form should be completd if an employee is willing to accept overtime for the stated six-month period if the need for overtime assignments should occur within their equalization unit.
