Apply for Benefits

To apply for Veterans benefits, take the following steps:

Submit or mail the following documents to the Veterans Office, Clark Hall, IUP Main Campus.

  1. Copy of the document that pertains to your benefit chapter (DD214, DD2384, or the dependent’s Certificate of Eligibility).
  2. Complete the Application Form which pertains to the Benefit Chapter to which you are applying. Select from the following:
  • VA 22-1990 Form — I am a veteran and am requesting first-time benefits.
  • VA 22-1995 Form — I am a veteran and am requesting a change of program or place of training.
  • VA 22-5490 Form — I am a child/spouse of a deceased or disabled veteran.
  • VA 22-5495 Form — I am a child/spouse of a deceased or disabled veteran and am requesting a change of program or place of training.

3. Complete the General Information Sheet.

4. College Fund Contract (Kicker) if applicable.

The application forms and General Information Sheet are also obtainable at the Veterans Affairs Office and can be obtained by contacting us by phone, mail, or e-mail.

You may also request to conduct this process by mail if visiting the Veterans Affairs Office is not convenient to you.

Transfer students who have received a G.I. Bill at another institution need to complete only the VA form 22-1995 and a General Information Sheet.
