Faculty Affirmative Action Forms

Affirmative Action Approval Packet A - Faculty: These forms must be submitted to the Office of Human Resources as a complete packet. Incomplete packets will be returned.
Affirmative Action Approval Packet B - Faculty: These forms must be submitted to the Office of Human Resources as a complete packet. Incomplete packets will be returned.
Affirmative Action Form A - Faculty: This form is to be initiated by the department for all available faculty positions at the time the position becomes available.
