Please complete the application for admission to the CCAC/IUP Collaborative Elementary Education Program.
Be sure to provide all the information requested:
- A one-page typed statement outlining your personal and professional goals as they relate to education and the teaching profession. Include your goals and/or experiences in urban, suburban, and/or rural schools.
- The envelope containing your recommendation form, which has been completed and sealed by a CCAC faculty member.
- Official transcripts from each college or university you have attended.
Sign and date the application before returning it, along with your goals statement, the sealed envelope containing your confidential reference and your official transcripts, to the IUP Admissions office.
