You must request a new waiver of the university Health and Wellness Fee each semester you are eligible. There are currently no automatic fees assessed for summer; therefore, no waivers are needed for summer. Also, part-time graduate students and doctoral dissertation students have no automatic Health and Wellness Fees assessed during regular semesters; therefore, there is no need for them to apply for a waiver. Full-time graduate students are assessed a partial fee, which may be waived.
Eligibility requirements are as follows:
- The student is commuting more than fifty miles one way from his/her address of record to the campus he or she is attending
- The student is on a student teaching assignment more than twenty-five miles one way from main campus
- The student is on an internship site more than twenty-five miles one way from main campus
Waivers and refunds are not possible beyond the current and immediately preceding semester. See IUP’s policy regarding fees and refunds.
The Health and Wellnes Fee is a mandatory university fee. Waivers are granted only for the situations described above. Waivers are not granted to students who request them because they have a family medical doctor or personal health insurance. Waivers are also not granted to students who merely do not plan to use the service.
The waiver form is only available on line through URSA (University Records and Systems Assistant). Follow this procedure to complete the form:
- Go to the URSA website.
- Log in to secure area using your Banner ID and PIN number.
- Select “Student Services and Financial Aid.”
- Select “Billing and Fee Information.”
- Select “Request for Health Service Fee Waiver.”
- Your request is transmitted electronically to the director of IUP Health Service for review.
