AFSCME, SCUPA, and PSSU employees enrolled in a PEBTF health plan and/or supplemental benefits are eligible for an employee contribution waiver through participation in PEBTF’s Get Healthy program.
The 2013 Get Healthy Health Assessment will be offered from Monday, April 1, through Monday, April 22, 2013. To be eligible for the Get Healthy waiver, you and your covered spouse/domestic partner must complete the Health Assessment each year.
In addition to completing the annual Health Assessment, if you and/or your covered spouse/domestic partner are classified as either “At Risk” or “Chronic,” there are additional steps you need to take by April 1, 2013, to continue to receive the waiver. Additional information on these steps and the Get Healthy Program is available on the PEBTF website.
