Guidelines to Administer Student Evaluation Instruments

A joint APSCUF and Management committee was formed to establish guidelines to administer student evaluation instrument.

IUP-APSCUF and Management agree to follow the attached Procedures for Administration and Use of Student Evaluations of Faculty as well as Directions for Administering the Student Evaluation Instrument, to be printed on the Evaluation Envelope.

Procedure for Administration and Use of Student Evaluations of Faculty


A.  Procedure for Administering Student Evaluations

  1. Approved Evaluation Instrument will be printed directly on OpScan answer sheets.  A supply of these forms will be provided by Management to the Chairperson or a designated faculty member of each department.
  2. The Student Evaluation Instrument shall not be administered sooner than the 12th week of a full semester’s teaching or at the 2/3 point of a given instructional period.
  3. The Student Evaluation will be administered by the Department Chairperson or a faculty member designated by the Chairperson.  Administration will take place in the classroom of the faculty member being evaluated, during the regularly scheduled class period.  The instructions given to students shall be only those printed on the Evaluation envelope (see Directions for Administering the Student Evaluation Instrument at the end of this document).
  4. The faculty member administering the form shall write, on the chalkboard, the following information:
    a. The evaluated faculty member’s name
    b. The course and section number
    c. Year and term code
  5. The faculty member being evaluated shall not be present in the room either while the instructions are being given, or while students are completing the forms.
  6. The faculty member administering the Student Evaluation shall return the evaluations directly to the Department Office, or in the case of a Branch Campus to the Administrative Office of that campus, in a sealed envelope.  The envelope seal shall be signed by the faculty member administering the Instrument and the sealed envelopes shall be secured until delivered to the Information Technology Support Center for computer processing.
  7. All data will be backed up and maintained by the University for 18 months and will be accessed only with consent of the faculty member, and the APSCUF and University Presidents in the event any problems with the evaluations need to be resolved.
  8. Any unused forms shall be returned to the Department office.

B.  Distribution and Use of Student Evaluations

  1. The Deans’ office will make available directly to departmental faculty designee, or if necessary to faculty member evaluated, all questionnaires completed by the students and four printouts of the Student Evaluation results.
  2. The above information will be distributed after the final grades have been submitted.
  3. The faculty member or departmental faculty designee will acknowledge the receipt of the Student Evaluation materials listed above when they are picked up at the Dean’s office.
  4. It will be the responsibility of the faculty member to make the Student Evaluation data available to the Department Development and Evaluation Committee and the department chairperson upon request during the evaluation process (see Art. 12).
  5. If a department has formally approved a set of Supplementary Departmental Items, results on those items shall be reported along with the University-wide items.  Such items may not supplant the information from the university-wide items.

  1. Complete the items in the box at the top of this envelope.
  2. Print the following information on the chalk board:

  3. Read aloud the following instructions:
     
    “This evaluation was designed to provide feedback to your instructor, and thereby aid him/her in the development of more effective teaching.  Results from the evaluation are used by departmental and campus-wide committees for making personnel decisions.  Thus, it is very important that you respond to each item in a careful and conscientious manner.  I will return the evaluations to the department office in a sealed envelope.  The evaluations, plus a computer summary of the results, will be returned to the instructor after the semester grades have been submitted.  There shall be no discussion of the evaluation while it is being administered.”
  4. Distribute one blank questionnaire to each student.  If the department has supplied additional questions for Section II, distribute these questions at this time also.
  5. Read aloud the following instructions:

    “The items on the front of this questionnaire will be scored by an optical scanner.  Therefore, it is necessary to use only a Number 2 pencil.  Please complete the information in the box in the upper left corner of Side One.  I have printed this information on the chalkboard.  Once you have answered all the questions on Side One, please complete the open-ended questions on Side Two.”
  6. When the students have finished, collect all the completed questionnaires.

    a.  Be sure that all questionnaires face in the same direction.

    b.  Remove all BLANK forms.  DO NOT ENCLOSE BLANK/EXTRA FORMS IN THIS ENVELOPE.

    c.  Place a completed “Student Evaluation Request” form (form to be completed
        earlier) on top of the questionnaires.  If this form is missing or not completed,
        notify the department chairperson.

    d.  Be sure the information at the top of this envelope is complete and accurate.

    e. Count the number of completed student evaluation questionnaires.  Enter this total below and place your initials next to this number.

    f.  Place the completed student evaluation instruments and the Student Evaluation Request Form in this envelope.

    g.  Seal the envelope and write your signature across the seal of the envelope before leaving the room in which the evaluation was administered.

    h.  Return this sealed envelope and any unused forms to the department chairperson.

DIRECTIONS FOR RETURNING AND SCORING STUDENT EVALUATION INSTRUMENTS

  1. The faculty member administering the student evaluation shall return the evaluations directly to the Department Office or in the case of a Branch Campus to the Administrative Office of that campus, in a sealed envelope.  The envelope seal shall be signed by the faculty member administering the instrument and the sealed envelopes shall be secured until final exam week when they will be delivered to the Information Technology Support Center for computer processing.

    Envelopes will be delivered to the Information Technology Support Center between normal university business hours. 
  2. All envelopes for a particular instructor will be delivered together.
  3. A “Production Run Sheet will be completed and attached to the packet of envelopes.
  4. When the envelopes are delivered to Information Technology Support Center, the department representative will be required to sign the “Production Run Sheet” indicating the accuracy of the number of envelopes per faculty member being evaluated.
  5. Failure to follow all instructions may result in invalid processing of the evaluations.  In such cases, the evaluation forms will be returned to the dean’s office for corrections.
  6. The Information Technology Support Center will send the scored evaluations to the dean’s office for distribution to the faculty.
