Health Forms

A current health form should be on file with the scoutmaster at all times and it should be updated annually. The scout or parent should notify the scoutmaster of any health information that would materially change the scout’s health record.

Effective in 2009, the Boy Scouts of America have instituted a new health policy and form and mandated that each scout and scouter have a new form completed by his or her physician each year. Beginning in 2010, only the new form will be acceptable for scouting activities. The health form is available from the scoutmaster, the advancement chair, or on line at the council or national website. There are provisions available for waivers of injections and medical treatments, should these requirements conflict with family-held or religious beliefs.

The new form includes a “Hold Harmless” provision, plus a “Talent” release (Part “C”) which, if signed by the parent, guardian, or Scouter, will permit the council to use a picture of the scout or scouter on troop, council, or national publications to promote scouting. (Amended 9/2009)

Photographs

Parents should be aware that photos taken during troop activities may be used on this website. Council policy requires that a signed permission form be on file with the webmaster before a scout’s picture can be used. Under BSA policy, individuals are not identified by name nor are activities identified by location in the Web page photographs.
