This form is to be used to apply for eligibility to teach doctoral and/or master’s courses. Eligibility for doctoral courses automatically provides eligibility for master’s courses.

Approval by the dean of the Graduate School and Research renders the applicant eligible to teach graduate courses. However, applicants must remember that final teaching assignments for a graduate course will continue to rest with the department chairperson and the dean of the college.

A copy of the duly completed application for eligibility to teach graduate courses will be submitted to the department’s Graduate Review Committee (GRC), which will be responsible for reviewing and verifying the information contained in the applications. The GRC’s recommendations, along with the application and the support material, will be forwarded to the department chair, who in turn will review and render individual recommendations and further forward the application to the graduate dean, who will make the final decision. Please note, as stated above, only one copy of the application needs to be submitted to the GRC chair, who will review the application and then forward to the department chair.
