The PEBTF annual health care plan open enrollment for AFSCME, SCUPA, and PSSU employees will take place from October 1 through October 19, 2012.

Open enrollment is your annual opportunity to select a new health plan, waive your health care coverage, add dependents or remove dependents from your coverage. Changes made during the open enrollment period will be effective January 1, 2013. Employees may enroll dependents outside of open enrollment; however, the effective date of coverage cannot be more than sixty days retroactive from the date the PEBTF-2 Enrollment/Change form is submitted to the Office of Human Resources.

Cost Information for 2013

Full-time employees hired prior to August 1, 2003, pay the employee contribution for health benefits through payroll deductions. The contribution rate has not changed for 2013. If you participate in the Get Healthy Program, you pay 1.5 percent of your salary. If you do not participate in the Get Healthy Program, you pay 3 percent of your salary. There is no additional cost to you, regardless of which pay you choose or how many dependents you have covered by your plan.

Full-time employees hired on or after August 2, 2003, also pay the employee contribution for health benefits as outlined above. The HMO and Consumer Driven Health Plan (CDHP) options are the least expensive plans and are offered at no additional cost to you. The Highmark PPO plan may be purchased for an additional biweekly cost of $21.38 for single coverage, or $54.08 for family coverage.

Part-time employees also pay the employee contribution the employee contribution for health benefits, plus an additional amount depending on the chosen plan and the number of dependents covered by your plan. For cost information, please refer to page 6 of the 2012 PEBTF Open Enrollment Newsletter.

Selecting a New Plan Option

To find out more information about the health plans offered, review pages 3-5 of the 2012 PEBTF Open Enrollment Newsletter, visit the PEBTF website, or contact the PEBTF at (800) 522-7279. If you elect to change your plan, you should expect to receive your new ID card(s) in late December. Your new plan will become effective on January 1, 2013. The new employee health care contributions for employees hired on or after August 1, 2003, will be effective with the pay you receive on January 25, 2013. Plan option changes may be made through Employee Self Service (ESS) between October 1 and October 19, 2012 or by submitting a PEBTF-2 Enrollment/Change form to the Office of Human Resources by October 19, 2012.

Adding or Removing Dependents from Your Plan

You may add or remove dependents to your plan during open enrollment with an effective date of January 1, 2013. For dependent eligibility information, refer to the PEBTF Summary Plan Description. Dependent children ages nineteen to twenty-six may be dependents on your plan if they are not eligible for health insurance through their employer, or, if married, through their spouse’s employer. To add dependents to your plan, you must provide supporting documentation of dependent eligiblity to the Office of Human Resources no later than October 26, 2012. Eligible dependents may also be added to your health plan outside of open enrollment; however, the effective date may not be more than sixty days retroactive to the date you sign the PEBTF-2 Enrollment/Change form.

To remain in your current plan with your current dependents, no action is necessary.
