Since ethnicity/race/sex identification is a critical reporting factor, data collection forms with no ethnicity/race/sex identification will not be processed.

Gender
Veteran Status

  1.      Any honorably discharged person who served in the armed forces of the United States, or any women’s organization connected with those forces, during any war or armed conflict, or who has served since July 27, 1953. This service in the armed forces includes training time and service in the state National Guard and the United States Army Reserve.

  2.      A veteran entitled to compensation (or one who was released or discharged from active duty) for a service-connected disability.
Ethnicity

  1.      Persons of Cuban, Mexican, Puerto Rican, South or Central American or other Spanish culture or origin, regardless of race.
Race

  1.      Persons having origins in any of the original peoples of North and South America (including Central America), and who maintain tribal affiliation or community attachment.

  2.      Persons having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian subcontinent, including for example, Cambodia, China, India, Japan, Korea, Malaysia, Pakistan, the Philippine Islands, Thailand, and Vietnam.

  3.      Persons having origins in any of the black racial groups of Africa.

  4.      Persons having origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands.

  5.      Persons having origins in any of the original peoples of Europe, the Middle East, or North Africa.
