This form is for IUP students holding an F-1 or J-1 visa only.
Unless you have had a previous semester with a reduced courseload request, please withdraw from your class after you have submitted this form. If you have already had a reduced courseload, you should wait for approval prior to withdrawing from a class.
Final Semester Deadline: by end of Drop/Add Period
Academic Difficulty Deadline: Semester Withdrawal Deadline
You must have approval before dropping classes. If you are below full-time WITHOUT permission—our office is to comply with immigration regulations and terminate your SEVIS record and you will be out of status (illegal). If you receive Partial Tuition Waiver—your waiver will be reduced to reflect your credit load: $100/credit for Undergraduates and $75/credit for Graduate Students.
This is not for Ph.D. students working on their dissertation; please fill out green form.
If you are in the middle of the semester, please wait to be notified of approval before you drop any classes.
