Below, you will find links to some frequently used applications and forms. You can save a trip to the dean’s office and simply print the forms on your desktop printer.

Application for Approval of Excess Academic Load: Required for students who wish to take 18 or more credits.
Application for Minor or Change of Minor: Required if you wish to add or remove a minor from your transcript.
Remove Minor or Major Form: Form to remove a minor, a double major or a dual baccalaureate degree.
Application for Dual Degree: How to apply for a second baccalaureate degree (rather than just a double major).
Change of Major Form: Change of Major Form

Exception of Liberal Studies: Form to petition for a Liberal Studies exception. Exceptions to Liberal Studies requirements may be considered only in extreme or unusual circumstances.
Overlap Approval Form: Under rare circumstances, students may register for two courses whose times overlap.
August Graduation Application: Undergraduate Application form for August Graduation.
