Registration

Registration is requested on or before November 1. Payment should accompany your registration.

Walk in registration is also welcome. However, we ask that you please e-mail us so that proper provisions can be made.

Continuing Education Credit

Healthcare providers will receive a certificate of attendance for this 6.5-hour presentation, which they may submit for continuing education units as per their license requirements.

COSM is an approved provider of Continuing Education Units through the Board of Certification, Inc. under approval number P973.

Application has been made for CE approval from the Pennsylvania State Board of Physical Therapy.
