How to Complete the Test Header for Test Scoring

One test header sheet is required for each group of general purpose answer sheets. Information that is required on this sheet is in the maroon area. The gray area is for optional information used for reporting.

It is recommended that you complete the name fields. These fields will appear on the reports.

Maroon Grids (required data)

The data in the maroon shaded grids on the left side of the form is required for processing. You must complete the grids as described below in order to have your exam processed.

  • Subject Course Section: Starting in the leftmost column, darken the bubbles representing the Subject, Course Number, and Section number for the course. Make sure to darken the blank bubble between each value. Remaining bubbles can be darkened as desired to convey information about the exam.
  • Sub Tests: The number of sub tests to be processed from this exam (maximum of 10). This number should equal the total number of key sheets submitted. Normally you should have a 01 in this grid.
  • Formula Scoring: Yes or No. If Yes, fill in the factor to be used in scoring (# of responses – 1). You must make a selection and darken the bubble. You must also complete the formula scoring section on the Test Scoring Request Form.
  • TSC Use Only: Items in these grids are used by the staff processing your exam. You should make no marks in any of these grids.
