How to Complete the Test Scoring Request Form

To submit an exam for scoring at the IT Support Center, you must complete the Test Scoring Request Form.

In the past, this has been printed on an envelope available through Central Stores. Due to a large supply of these envelopes, they will continue to be used to package the exams, but only the requestor’s information must be completed on the envelope. You must also print and complete the new Test Scoring Request Form (effective Spring semester 2012), and include it inside the envelope with the exams.

All information should be clearly printed!

Requestor’s Information

  • On the envelope, print your name, IUP username, department, and phone number in the appropriate spaces. This information is required. You will only be called if the envelope has a missing or incomplete answer key or header sheet. When exams are completed, you will receive an e-mail notification rather than a phone call. Exams are processed on a first-come, first-served basis, so desired completion date is not necessary.
  • On the Test Scoring Request Form, print your name, IUP username, department, and phone number.

Scoring Information

Identify the scoring options you wish to use. You can read more about each option by clicking on the appropriate link.

Standard Exam

One point per question, one version of the exam, one response per question, no penalty scoring, no bonus questions, no subtests.

Exams with Multiple Versions

This option allows you to process multiple versions of an exam in the same batch, providing one set of reports and one results file. If you wish to have a report for each version or a results file for each version, the exam must be processed separately.

Multiple Point Values

This option allows you to assign different point values to questions on the exam (weighted questions).

Formula Scoring

This option allows you to penalize student for wrong answers. A student will receive the designated point value for a correct answer, but a fraction of the point value is deducted for an incorrect answer. If the student doesn’t respond to the question, there is no deduction.

Bonus Questions

This option allows you to identify up to six questions as bonus questions. A student will receive the designated point value for a correct answer, but the bonus questions are not included in the total possible points.

Multiple Responses

This option allows you to identify an exam that requires multiple responses. There are two types of multiple response options:

  • AND — Where the student responses must match the responses on the answer key exactly. All responses must match.
  • OR — Where the student must respond with one of the answers provided on the answer key.
Subtests

This option allows you to evaluate subtests—or learning objectives—on an exam by identifying questions for each learning objective. If questions 1 through 10 on your exam deal with learning objective A, 11-20 deal with learning objective B, 21-40 deal with learning objective C, and 41-50 deal with learning objective D, you can use subtests to obtain a score for each learning objective for each student. These scores are only available on the Student Statistics Report. On the individual Student Grade Report, only a total score is provided. Subtest scores are not included in the results file.

Report Information

By default, no reports will be printed. If you wish to have any of the five reports printed at the IT Support Center, please select the appropriate check boxes.

You can review the Test Scoring Reports with descriptions and sample reports. All available reports for your scoring selections will be generated and placed in a folder that only you can access. A link to this folder will be sent to you in your e-mail notification when the exam is completed. You also have the option of printing these reports to your own printer.

Results (Data) File

There is no longer a section to request a data file. By default, a results file in Excel format will be created for every exam that is processed. This file is placed in a folder that only you can access. A link to this folder will be sent to you in your e-mail notification when the exam is completed.

Final Steps

When you have completed the front of the envelope (ID information only) and the Test Scoring Request Form, you will arrange the completed test scoring sheets in the envelope in this order:

  1. Test Header
  2. All answer key sheet(s) in numerical order
  3. Answer Sheets (GPAS)

Once the sheets are in order, place them in the envelope and securely seal the flap. The envelope can then be taken to the IT Support Center located on the ground floor of Delaney Hall.
