The general purpose answer sheet (GPAS) is the sheet students use to record their answers to the test questions. Instructors will need to tell the students how to fill out the header area.
The answer sheet should be completed as follows:
Name
- Last Name, FI (First Initial), MI (Middle Initial) should be entered one letter per column. The corresponding "bubbles" for each letter should be darkened. The information in these bubbles will be printed on the reports. An IUP computer account username can also be used to identify the student’s test.
Student ID
- This must be a unique number at least two digits in length.
- It must begin in the left-most column of the Student ID grid.
- The IUP Banner ID is used at the instructor’s discretion. If this number is used, it must start in the left-most column of the student ID grid and begin with a number—NOT an @ sign.
- No two students can have the same ID number on a GPAS.
Seat Number
The seat number field is used to identify the version of an exam. If you are administering an exam with multiple versions, you will need to instruct the students on the correct way to complete this field. See How to Submit an Exam with Multiple Versions for Test Scoring. If you are not using multiple versions, this can be left blank.
For Best Results
Ask the students to use the following guidelines:
- Use only a No. 2 pencil. If anything else is used to darken the bubbles, the marks cannot be read by the scanner.
- Make heavy black marks that fill the circle completely.
- Erase thoroughly any answer to be changed.
- Make no other stray marks anywhere on the answer sheet.
- Fill in the heading information as instructed by professor.
