International Student and Scholar Request Form
Application to Replace I-94: The I‐94 card (white card) is the ARRIVAL/DEPARTURE record used by Department of Homeland Security to track who has entered and exited the U.S. If you would like to replace your I‐94, please follow the steps.
Reporting U.S. Address Changes for Immigration Purposes: International students should complete this form to comply with USCIS regulations concerning immigration.
Conversation Club Application: Sign up for Conversation Club, which pairs up international students with native speakers to learn and exchange language and culture.
Curricular Practical Training Application: People who are planning to take an internship need to complete this request form.
Drivers License and PA Identification Card Instructions: The Pennsylvania Departmeen of Motor Vehicles (DMV) oversees issuing of drivers licenses and state identification cards. Most non-immigrants and their dependents can obtain a Pennsylvania (PA) drivers license or state identification card.
International Friendship Program Student Application: An application for IUP students who want to spend time with an American family.
J-1 Academic Training Application: Academic training is work, training, or experience related to a student’s field of study and offered through the Office of International Education International Student advisor. Partial Tuition Waiver Appeal
Partial Tuition Waiver Hour Confirmation Sheet: People who ARE doing PTW need to complete this form to confirm their hours.
Program Extension Form: When you need to continue in an educational program beyond the originally estimated completion date, you must apply for an extension no later than thirty days before the expiration date on your I‐20 or DS‐2019.
Readmission Form: If you are a readmission student, please complete this form and return it to the Office of International Affairs for appropriate immigration documents. (PDF, 52 KB)
Reduced Courseload Authorization
Reporting OPT Employment For SEVIS: Students on OPT have to report changes in US address and Employer address and name to OIE to update their SEVIS record.
Social Security Work Authorization Form: All new incoming international students can be issued a Social Security number only when they have been hired for a job. Bring this form to your employment supervisor for a signature and then to the Office of International Education for a signature before visiting the Social Security office. (PDF, 93 KB)
Verification of Status for Master's and Doctoral Students: If you are not registered for dissertation credits but are completing an M.A. thesis or preparing for comprehensive exams, please complete this form. (PDF, 42 KB)

Forms for Prospective International Students

Undergratuate and ALIP Affidavit of Financial Support: Complete this form to demonstrate your ability to pay for tuition, fees, and other expenses during your undergraduate studies at IUP.
International Transfer Student Form: Students studying in the U.S. or on Optional Practical Training must have the foreign student advisor complete and return form to IUP. I-20s will be issued after receiving form and SEVIS transfer date.
