The Office of Housing and Residence Life makes use of the following forms:

Housing Agreements and Contracts

Agreement Contract, Apartment: IUP Office of Housing and Residence Life Housing License Agreement for Apartment Living, 2010-2011 Academic Year. This is a legal agreement between IUP and the individual student and cannot be cancelled once finalized.
Agreement Contract, Residence Halls: IUP Office of Housing and Residence Life Residence Hall Housing License Agreement and Dining Services Contract, 2010-2011 Academic Year. This is a legal agreement between IUP and the individual student and cannot be cancelled once finalized.
Agreement Contract, Suites: Suite Housing License Agreement and Dining Services Contract 2010-2011 Academic Year is a binding contract. The Foundation for IUP has partnered with the university to develop the Suites at IUP. IUP is the exclusive manager of these facilities.

Student Dining Service Policy: Dining Agreement terms
Off Campus Summer Dining Plan: Dining plan contract for summer for off-campus students, to be returned to the Office of Housing and Residence Life. On-campus students must select a dining plan on their housing contract.

Food/Alcohol/Refreshment Request Form: Food and Alcohol Request Form. File at least one week in advance of event.
Meningitis Compliance Form: IUP Meningitis Policy compliance form, required by law
Posting Procedure: Posting of bulletins is coordinated by Office of Housing and Residence Life.
Residency Requirement Exemption Request Form: Form used to request exemption from two-semester Freshman residency requirement at IUP
