Services and Fees

We do not bill insurance companies. Any charges incurred will be posted on your IUP URSA account, and it is your responsibility to pay through EasyPay. We are able to provide you with a billing statement that you may submit to your insurance company for possible reimbursement.

Beginning with the fall 2015 semester, the cost of services is no longer based on the wellness fee. As long as a wellness fee is paid, all students will be charged the same, whether undergraduate or graduate student.

No-show visit fee

Medications are not included in the visit fee. Students may either purchase medications at the Health Service, or may ask for a prescription. Any medications that are taken out of the Health Service will be charged to your student account, and refunds are not provided.

No show visit fee:

If you have scheduled an appointment at the Health Service and cannot attend, you must cancel the appointment. You will be charged a $25 fee for each missed appointment. You can avoid this charge by canceling your appointment. Non-cancelled appointments are missed opportunities for other students in need of our service.

Special Services

  • Women’s yearly gynecological exams: $35. Laboratory tests are not included in this fee and will require insurance information, or payment of the lab costs will be billed to you directly from the lab. If STI testing is performed during this exam, there will be an additional $15 charge.  Oral contraceptive services are available and vary in price.   • Physicals: Immunizations records are required for all physicals, except driver’s license physicals. Physicals are done by appointment only.
  • Immunizations: (More info) A record of your immunizations is required. Paperwork for insurance reimbursement will be provided.
  • Ambulance transportation: Transportation to Indiana Regional Medical Center is available through Citizens Ambulance.  Your insurance will be billed for this service. If you do not have insurance, you will receive a bill from Citizens Ambulance.
  • Van transportation: This service is offered free of charge from the Health Service to doctor appointments within three miles of the Indiana campus, provided it is during normal business hours.
