Services and Fees for Fall 2008 and Spring 2009
Health Fee A—$150 per semester
All full-time undergraduate students pay this mandatory fee. This fee is only able to be waived in certain circumstances. (Find more info about waiving the fee.)
Health Fee A covers the following:
- Most of the services provided by physicians, nurse practitioners, and registered nurses, including procedures and supplies
- All programs and services listed in Fee B
- Most health center visits involving a physician, nurse practitioner, or registered nurse will incur a $10 co-pay per visit.
- Self-care cold and wound clinic supplies and medicines (no charge)
- TB testing is free for Fee A if using mass clinics; there is a $15 fee if not during clinics.
- Pregnancy testing alone ($10, plus $10 co-pay)
- Emergency contraception only (Available without a provider visit.) There is a $25 medicine charge, with no co-pay. Sign in and request from front desk.
- Minor laboratory tests performed at University Health Service (no additional charges)
- Woman’s yearly gynecological exams (Laboratory tests are not included in health fee and will require insurance or payment of lab costs. Co-pay is $10.)
- Ambulance transportation to Indiana Regional Medical Center at no direct cost to you or your family
- Eligibility for allergy clinic injections (serum provided by student) are available for Fee A payers at a cost of $35 per semester.
- Eligibility for the student health insurance plan. Must be an enrolled graduate student (one or more credits) or an enrolled undergraduate (six or more credits).
- Fee A will be charged to all enrollees of university required student health insurance.
Health Fee B—$30 per semester
This partial fee is mandatory for all part-time undergraduates and all full-time graduate students. The fee is able to be waived in certain circumstances. (Find more information about waiving the fee.)
Health Fee B allows the following:
- Use of the Self-Care Cold and Wound Center (no charge)
- Flu shots (no charge)
- Van transportation to health care appointments within three miles of the Indiana campus (no charge)
- TB testing, $15
- Pregnancy testing, $30
- Emergency contraception only (Available without a provider visit). There is a $35 medicine charge with no co-pay. Sign in and request from the front desk.
- Physicals, $30. Call to inquire if the physical you need is offered at the health center (no additional co-pay).
- Access to programs and services provided by Health Education and Advocacy Office and the Office of Alcohol, Tobacco, and Other Drugs
Note to Fee B payers: You are eligible for the student health insurance plan when you upgrade to Fee A. You must be an enrolled graduate student (one or more credits) or an enrolled undergraduate (six or more credits). If you enroll in the plan, you will need to upgrade to Fee A.
Fees for Special Services
- Meningococcal meningitis vaccination, $90. No appointment needed.
- Physicals, $30 (teacher certification, athletic, child development, driver’s license, SCUBA). Check to see if specific physicals are provided. No appointments are used; physicals are done on a walk-in basis.
- STD testing lab fee is $15, plus $10 co-pay. Covers visit and laboratory work ($45 if the student has not paid Fee A).
- Medications (varied). Medications require a co-payment by the student that varies depending on the product. Students may also request a prescription that can be filled at the pharmacy of their choice. The health service does not operate a pharmacy. We carry a select inventory of medications (“formulary”). Prescriptions from medical providers outside of the health service cannot be filled by our office.
- Records fee, $15. Student interns can have their records reviewed and stored at the health center for internships that require medical data.
- Visit fee, $30. Students who have not paid Fee A may pay per visit if they enroll in Fee B and choose to be seen by a provider. Medications are not included. There is no co-pay with this fee.
A portion of the health fee (both Fee A and Fee B) supports the IUP Health Education and Advocacy Program, the Center for Counseling and Psychological Services, and the Alcohol, Tobacco, and Other Drugs Program. The Health Education and Advocacy Office is in Pratt Hall, Room 106. The ATOD Office is in Pratt Hall, Room 108. The Center for Counseling and Psychological Services is on the third floor of Pratt Hall. All IUP students are eligible to participate in the programs and services offered by the Center for Student Life.
