All IUP students must complete a health history and immunization form. The information contained in the forms listed below is now able to be entered electronically via this weblink by the student. Do not send paper copies of this information. Contact us by e-mail or telephone if there are any questions.
We realize many students have submitted this information previously on paper, and the Health Service will continue to maintain those records. However, this method will allow easier, more timely access to your health history and immunization status in the event you visit the Health Service.
Only currently registered students will be able to submit information to their electronic medical records via this link. IUP students must use their user name and password to enter the site.

Once on the page, read the instructions and submit the information only when you have completed the form. This page will also allow you to view and print the immunizations that you have on file with the Health Service whenever you choose.
