Declaration of Disability: Please be aware that IUP does not discriminate on the basis of race, gender, age, national origin, religion, sexualpreference or disability in any of its educational programs or activities. We provide services commensurate with Section504 of the Rehabilitation Act and the Americans with Disabilities Act. You should register with this office even if theBlind and Visual Services (BVS) or the Office of Vocational Rehabilitation (OVR) is servicing you, as we will worktogether to provide you with support services.

Recorded Books Form #1: Please submit this form for all recorded book requests EACH SEMESTER. You will need your course syllabus to complete the form, and you will also need the ISBN for the book you are requesting. Once the form is complete, please return it to Advising and Testing, 216 Pratt Hall.
Recorded Books Form #2: Please submit this form after it has been determined that your textbook(s) must be locally recorded. If you are unsure if your textbook must be locally recorded, please contact Matt Mascari at 724-357-4067 or m.r.mascari@iup.edu.

DSS Test Request Form (updated 3-10): Use this form to request a test within Disability Support Services. Form must be downloaded, completed and returned to Neal Stidham (n.a.stidham@iup.edu) at least three business days in advance.
Documentation of Disability Form: This is a form for students wishing to verify a diagnosis of Attention Deficit Hyperactivity Disorder, Psychological/Emotional Disorder, Health/Physical Disorder, or Mobility Disability. The form must be completed by a doctor, psychologist, or other appropriate professional. This form is optional; a letter from the professional may be sufficient, as long as it includes what is on this form. This form cannot be used to document Specific Learning Disabilities, Traumatic Brain Injuries, Vision Impairments, or Hearing Impairments.
