These are forms each prospective client of the SBDC needs to complete in order to receive the counseling services offered by the SBDC.

Request for Counseling (electronic form below)

Please complete and submit an online Request for Counseling, then contact the SBDC to schedule an appointment to discuss your business assistance needs. Print, sign, and bring the Client Rights and Responsibilities and Publicity Release forms with you to the appointment.

Each client should have a signed Client Rights & Responsibilities form on file before beginning the counseling engagement.  If you have not already turned one in to the SBDC, please print and bring one with you to your next counseling session.

The SBDC requests a signed Publicity Release Form prior to releasing any details for public information or educational purposes.
PREP Waiver of Confidentiality Document icon
The Pennsylvania Small Business Development Centers maintain strict confidentiality relative to all matters relating to its clients.  Because there are other economic development growth programs, some clients may benefit from a collaboration of these programs and the Small Business Development Center (SBDC).  The only condition under which a Small Business Development Center can share client information with other organizations is if the client provides explicit written permission to do so.  An SBDC consultant will ask a client to sign this form if collaboration will be beneficial for the business.  If you completed the online Request for Counseling, this form is not necessary.
