Direct Deposit Cancellation Form 0910: Fill out this form to cancel direct deposit of your financial aid refund for the 2009-2010 school year.
Employer Tuition Payment/Reimbursement Form: Complete and submit this form along with any payment due that is not reimbursed by your employer.

Financial Hold Override Form: Students with a financial hold who may need to remove classes from their schedule can complete and return this form to the Registrar/Scheduling office in Clark Hall, Room 321.
Stop Payment Request Form: This form needs to be completed any time a student wants a stop placed on an outstanding check.
Tuition Waiver Form: To be completed if courses are to be taken at IUP and you are eligible for a tuition waiver
