Financial Hold Override Form Document icon
Students with a financial hold who may need to remove classes from their schedule can complete and return this form to the Registrar/Scheduling office in Clark Hall, Room 321.
Stop Payment Request Form Document icon
This form needs to be completed any time a student wants a stop placed on an outstanding check. 
Title IV Authorization Document icon
By completing this form, you are giving permission to the university to pay any outstanding balance on your student account (including non institutional charges) for the same aid year.  This will not cover charges from prior aid years.
Tuition Waiver Form Document icon
Tuition waiver forms must be submitted for each session.  Complete the entire form and submit it to the Office of the Bursar prior to the beginning of each session.
Direct Deposit Cancellation Form
Direct deposit cancellation forms are no longer necessary. If you wish to discontinue the direct deposit of financial aid refunds, please utilize URSA to do so.
Employer Tuition Reimbursement Form Document icon
Complete this form and submit it along with any remaining balance due to the Office of the Bursar.  Please download and submit a new form each semester.
